Kindergarten Registration

BEGINS ON MARCH 23 AT ALL ELEMENTARY SCHOOLS!

Parents with a child who will be 5 years old on or before January 1, 2010 should contact their neighborhood Elementary School to give their name and address to the school secretary. She will send a Kindergarten registration packet that includes all necessary forms to you in March. The attached “Guide to Kindergarten Registration” lists the information that will be needed.

Norwalk Public Schools provides a FREE booklet entitled: “Kindergarten Readiness and Expectations: Getting Your Child Ready for School.” Anyone may request a copy by calling Mary Budrawich at 854-4147.

Questions may be directed to any Elementary School office or you may call Mary Budrawich, Early Childhood specialist at 854-4147. Parents may also call Mary if they do not know which school their child will attend.

REGISTRACIONES PARA KINDERGARTEN

Las registraciones para Kindergarten comenzarán el 24 de Marzo en las escuelas elementales de Norwalk.

Padres que tengan niños/as que cumplan 5 años de edad para el primero o antes del primero de Enero deben comunicarse con la secretaria de las escuela de su distrito para darle su nombre, y dirección correta y ella le enviará en Marzo las formas necesarias que usted necesita elenar.

La guia de información de las registraciones de Kindergarten adjunta, le ofrece una lista de lo que usted necesita para la registración de su hijo/a en Kindergarten.

Las escuelas públicas de Norwalk ofrecen gratis un folleto titulado: Preparación y Expactaciones - Preparando a su hijo/a para comenzar el Kindergarten. Todo padre puede solicitar una copia llamando a la Sra. Mary Budrawich al teléfono 854-4147 (en inglés).

Si tiene preguntas llame a una de las escuelas elementales o a la oficina de la Sra. Mary Budrawich, Especialista de Educación Prescolar, al 854-4147. Tambien puede llamar si no sabe a que escuela debe de ir su hijo/a. Preguntas? Llamar a Annette Esposito al 854-4091 ó 852-4314.

NORWALK PUBLIC SCHOOLS GUIDE TO KINDERGARTEN REGISTRATION

The following items are necessary for the registration of your child in the Norwalk Public School Kindergarten Program:

1. A certified birth certificate or other official evidence of birth date (i.e. passport)

2. The completed Enrollment Form (YELLOW)

3. The completed Developmental Questionnaire (PINK)

4. The completed Home Language Survey (GREEN)

5. The completed Transition to Kindergarten Preschool Information Checklist

(filled out by staff of preschool your child attended)

6. The completed Student Information Emergency Card

7. Two Proofs of Residency in Your Name

Mortgage Statement/Lease Agreement

Tax Statement

Utility Bill (electric or telephone)

Note: A driver’s license is not an accepted proof of residency.

8. State of Connecticut Health Assessment Record (BLUE Form) - Evidence of physical exam done within one year of entering kindergarten and recorded

on the Blue State of Connecticut Health Assessment Form. - Evidence of adequate immunization.

4-DPT (the last dose must be given after the 4th birthday)

3-Polio (OPV and or IPV the last dose must be given after 4th birthday)

2 Doses Measles containing vaccine upon entrance to kindergarten

1 Dose of Mumps and Rubella vaccines given at age one or older

1-HIB (required if student is not age 5 at time of school entry)

3-HBV (Hepatitis B)

1-Dose of Varicella or proof of immunity

- Assessment of risk for tuberculosis.

Completed forms with requested documentation must be hand-delivered to the office staff of the elementary school your child will be attending.

Los siguientes documentos son necesarios para matricular a su hijo/a en el programa de kindergarten de las Escuelas Públicas de Norwalk:

1. Un certificado de nacimiento con un sello u otra evidencia oficial de la fecha de nacimiento (i.e., pasaporte).

2. Formulario de matrícula completo AMARILLO

3. Las preguntas del desarrollo completamente contestadas. ROSADO

4. Las preguntas del idioma del hogar completamente contestadas. VERDE

5. La información de la transición de la preescuela al kindergarten contestada por un miembro de la preescuela que su niño/a asistió.

6. El formulario de emergencia completo.

7. Dos (2) pruebas de residencia con su nombre.

Una prueba hipotecaria ó un contrato firmado por el dueño de la casa donde vive y el número de teléfono. Una prueba de pagos de tarifas (taxes/recibo) Un recibo de pago de teléfono, electricidad. (Nota: Una licencia de manejar no es prueba de residencia).

8. Requisitos de salud para nuevos participantes. (AZULE)

1. Evidencia de un exámen físico hecho dentro del año de haber entrado y ser registrado en el Departamento de Salud del estado de Connecticut en el formulario azul.

2. Prueba de vacunas necesaria:

4 DPT (la última dosis debe ser administrada en el cuarto ó después del cuarto cumpleaños).

3 Polio (OPV y/ó IPV – la última dosis debe ser administrada en el ó después del cuarto cumpleaños.

2 Dosis del sarampión conteniendo la vacuna cuando entra al kindergarten.

1 Dosis de la vacuna de las paperas y rubella administradas a 1 año o más.

1 HIB (se requiere si el niño/a no tiene los 5 años cuando empieza la escuela).

3 HBV (Hepatitis B)

1-Dosis de la Varicella

-Evidencia del exámen de TB y la documentación de resultados dentro del un año de ingreso

EL FORMULARIO COMPLETO CON LA INFORMACIÓN REQUERIDA DEBE SER ENTREGADO PERSONALMENTE A LA OFICINA DE LA ESCUELA QUE SU HIJO/A VA A ASISTIR.
